Forms and Other Information You Need
How the Process Works:
Step 1: Make an appointment by calling (734) 854-7061.
Step 2: After scheduling, download and print the appropriate forms listed below.
Step 3: Complete the forms in pen.
Step 4: Bring the following items with you to your first appointment:
a. the completed forms
b. your insurance card (if you have insurance)
c. your driver's license or other identification
Step 5: Co-pays are due at time of service; we accept cash or checks only.
Step 6: Get directions (see link below) and arrive 10 minutes early.
Step 7: Enter the lobby at the front of the building. You will see our door on the left.
Adult Patients: Save Time By Filling Out These Forms at Home
Child or Adolescent Patients: if a child or adolescent receiving therapy has NOT reached his or her 18th birthday, please use these forms:
