How to Set an Appointment:

Step 1: Call us at 734-854-7061, 419-500-0089, or 517-486-4005. If you are not sure who you want to see, we will gladly help you find the best match!

You will need your insurance information when you register.

Step 2: Bring the following items with you to your first appointment:

             a. your insurance card (if you have insurance)

             b. your driver's license or other identification 

Step 3: Co-pays are due at the time of service; we accept cash, checks, or credit card.

Step 4: Get directions (see links below) and arrive 10 minutes early.

Step 5: Lambertville: Enter the lobby at the front of the building. You will see our door on the left.

            Blissfield: Use the first entrance to the Educational Wing of the Church just off East                         Jefferson Street. As you enter the door, we are down the stairs; waiting room is first door on              the right.

Jackson: Our office is in the Commonwealth Commerce building downtown Jackson. Enter
the building and use the elevator to the 3rd floor. We are Suite 340-B.

Perrysburg: If you approach from the west (downtown Perrysburg), use the 2nd parking
lot entrance. If you approach from the east (from the direction of I-75), enter the first
parking lot entrance. Christian Care Connection’s office is located at the eastern corner of
the building.
